Working with clients suffering from personality disorders can be among the most challenging for therapists. There are several ethical challenges that can arise when making treatment decisions and it is important to ensure that those treatment decisions are ethically informed and based on available research findings.

Join Kathy Steele for a series of lectures on ethical challenges in the treatment of Personality Disorders. Three monthly groups of 2 hours each will focus on selected clinical topics chosen by Kathy as being particularly clinically relevant.

The overall purpose of this program is to encourage and help participants develop a better understanding of and manage the ethical issues and dilemmas they encounter in practice. To enable participants to increase their ability to recognize ethical issues and to apply ethical decision making frameworks and protocols through enhanced use of critical thinking skills.

01 SESSION
Ethics and the Treatment of Personality Disorders

Now On-Demand!

We will explore some of the major ethical issues in working with clients who have personality disorders. Many clients with complex trauma have either trait of or definite personality disorders. We can best understand these disorders as involving chronic disruptions in emotion regulation and motivation to change; unstable sense of self;  relational difficulties; rigid cognitions and schemas; and behavioral problems. Ethical issues include how to manage dangerous behavioral problems without rescue or punishment; how to manage intense countertransference; how to cope with intense demands on the therapist, and how to maintain boundaries and a therapeutic frame. Many case examples will be offered.

Participants will be able to:

  1. Describe the basic characteristics of personality disorders that lead to ethical dilemmas in treatment

  2. List at least one (1) ethical dilemma in treating personality disorders in traumatized clients

  3. Apply at least five (5) strategies to maintain an ethical stance in the treatment of personality traits and disorders.

11:00 am - 12:00 pm

02 SESSION
Ethics and Dissociation

Thursday, November 21, 2024

11:00AM - 1:10 PM ET (Now with 1 YEAR access to the recording)

This webinar will focus on some of the major ethical dilemmas in the treatment of Dissociative Identity Disorder. We will discuss ethical decision making about whether, when and how to work with dissociative parts in ways that promote the highest level of functioning. Clients with complex dissociative disorders report difficulties with voluntary awareness and control of behaviors, so we will discuss ethical issues regarding the responsibility of the client both in and out of treatment. Many ethical issues arise when one part of the client communicates with the therapist without inner consent of the whole person. The role of family and other support systems can be wonderful for many clients, but for some it may be destructive and maintain dissociation. We will explore ethical approaches to these support systems. Finally, issues of safety of self and others will be discussed in the context of helping dissociative clients take more responsibility while acknowledging difficulties with amnesia.

Participants will be able to:

  1. Describe times when it is important not to work with dissociative parts.

  2. Describe at least two (2) ethical challenges and solutions when clients do not recall their behavior.

  3. Apply  at least one (1) intervention with dissociative parts of self that promote integration.

11:00 am - 12:00 pm

  • Introduction

  • Time when we may not want to or cannot work with parts

  • Ways to work with parts that maintain boundaries and the active involvement of the client therapy

  • Ways to manage inner conflicts within the client when one part wants the therapist to have information and others do not.

12:00 pm - 12:10 pm

  • Break

12:10 pm - 1:10 pm

  • When support systems are part of the problem: Identifying and working with problematic enmeshed relationships outside of therapy

  •  Managing self-harm, suicide and other urgent/emergency behaviors without rescuing.

  • Q&A

03 SESSION
Ethics and Attachment

Friday, December 13, 2024

11:00AM - 1:10 PM ET (Now with 1 YEAR access to the recording)

We typically think of “attachment” as the heart of psychotherapy. But sometimes we over-focus on techniques and pay little attention to the relationship. Or we may miss enactments in the relationship that tell us something about the client’s experience. We may fail to attune to nuances of different attachment problems in our clients, and sometimes in ourselves. We will explore times when “attachment” is overwhelming and when the client pulls for more and more availability of the therapist. We can think of attachment as needing some regulation and explore the ethics of how we offer it, titrate it, and talk about it with our clients. This presentation will explore issues of “too much” and “too little” connection with the client, and the types of connection that may be more beneficial than others.

Participants will be able to:

  1. Identify at least two (2) ethical issues regarding attachment in therapy

  2. Apply at least one (1) collaborative strategy to offer the most effective attachment experience for the client

  3. Identify at least 0ne (1) of the choice points in therapy regarding attachment strategies.

11:00 am - 12:00 pm

  • Introduction

  • Neglecting the therapeutic relationship in favor of techniques

  • Enactments in the therapeutic relationship and their ethical management

  • The importance of understanding the attachment style of both therapist and client with clinical implications

12:00 pm - 12:10 pm

  • Break

12:10 pm - 1:10 pm

  • Ethical issues in caretaking and rescue

  • Ethical issues in avoiding the relational strategies and needs of our clients

  • Q&A
