Course Description

This program will explore the differences in therapy with the older client (>60) who has a significant history of trauma. First, we will delve into memory issues: Is it dissociation, normal age-related forgetfulness, or dementia? Given that there is an increased risk for dementia with traumatic life events and chronic mental health difficulties, these differences are crucial to assess. Next, we will discuss managing the tendency toward increased depression and anxiety in older clients, due to life circumstances and changes in the brain. How is therapy different in clients who have much more past than future, for whom the need to live well in the now is paramount? How can we determine how fixed the client’s coping strategies are, and whether we need to shore up whatever capacity they have, or whether we can safely delve into the past?

Developmentally, older clients are not focused on creating a future template, but instead are focused on taking stock, grieving what was and what cannot be, and are more acutely confronted with existential issues of mortality, control, suffering, and the like. A complication is that many older clients are confronted with compounding losses in the present: of health, meaningful work, cognitive capacities, increased vulnerability and loss of control, friends and family, increasing challenges in doing the tasks of daily life, and more isolation and loneliness. We will explore these issues of loss, and of existential anxiety, including when to focus on resources and when to help clients grieve.
